What is the Redwood Community Health Coalition (RCHC): Redwood Community Health Coalition (RCHC) is a network comprised of 17 members, 16 community health centers and wellness education site, and a total of 73 sites in Marin, Napa, Sonoma and Yolo Counties. Formed in 1994, our mission is to improve access to and the quality of care provided for under-served and uninsured people in our four counties. RCHC’s major focus for the next three years is to strengthen the capacity of member health centers to act as a regional network of comprehensive primary care that provides access to consistently high quality clinical care, effective care coordination and reduce health disparities. RCHC history: RCHC grew from an informational Sonoma County “Council of Clinics” monthly meeting in the early 1990s designed to share information and coordinate efforts. Led in large part by County of Sonoma leadership, the meetings evolved into a robust collaboration among health center directors around a shared goal of improving health care access and quality for underserved populations. RCHC officially incorporated as a non-profit organization in 1994 to received joint grants and implemented projects of mutual benefit to all health centers. Originally incorporated with health centers from Sonoma and Marin Counties, the coalition grew to add Napa and Yolo County Health Centers. Who our patients are: Our member health centers provide patient-centered primary care health homes to over 230,500 patients, including 137,700 Medi-Cal patients and nearly 54,500 uninsured individuals. Over 53% of our patients are Latino and 42% of our patients are best served in a language other than English. The vast majority of health center patients are low- and very-low income with 49% of patients having incomes below the federal poverty level (FPL) and only 8% of patients with incomes over the 200% FPL. Of the individuals served by RCHC community health centers in 2018, about 55% were living in Sonoma County, 20% in Marin County, 13% in Napa County and 12% in Yolo County.
